Why is home health care important?
Home health offers you specialized services in the comfort of your home to help with:
- Recovery from injury, illness and surgery through rehabilitation therapy
- Staying in your home longer and reducing hospitalization through medication management and administration, patient education, wound care, pain management and lowering your risk of being exposed to infections
- Managing chronic conditions such as heart disease, diabetes, stroke, arthritis, urinary incontinence or respiratory disorders

What is the difference between home health and home care?
Although they may sound similar, home health services focus on providing you with medical assistance and rehabilitation therapy – including medication management, rehabilitation services, wound care, social work and other medical treatments.
With home health, you receive care in your home from nurses, therapists and aides who are ready to help you reach your goals.
Home care services provide you with daily routine activities not associated with medical care. This can include everything from personal care like grooming, bathing and dressing to light housekeeping, shopping and companionship.

When it comes to referring your patients to a home health care provider, quality matters. We are devoted to helping people successfully live in their homes for as long as possible.
Our team of nurses, therapists and specially trained aides supports patients physically, emotionally and spiritually.
