Complete the form on a participating location’s Send a note link. You can connect with both residents and staff members. Include their name in the field, and we’ll print and deliver your message. If you would like to use Send a note to brighten the day of someone you don’t know, please put “any resident” or “any staff member” in the name field. We’ll make sure your message is delivered to someone who would love to receive a note.

Send a note is available at skilled nursing facilities, assisted living, senior-living – independent living and affordable housing locations.
